Our Services
The Regional Geriatric Assessment Program (RGAP) is a coordinated network of specialized assessment, treatment and rehabilitation services for elderly persons whose well being, independence or functional ability is threatened by multiple health or social problems. The RGP is hosted by the Ottawa Hospital and is a product of combined efforts of SCO Health Service (Saint Vincent Pavilion and Elisabeth Bruyère Pavilion), Ottawa Hospital, Queensway-Carleton Hospital. Geriatric Assessment Outreach Teams - top of page
The outreach teams provide a multidimensional in-home assessment in order to identify appropriate services and support for seniors with complex health problems. Our teams work with clients, their physicians, and caregivers as well as other health care providers to optimise seniors' independence and quality of life.
Geriatric Assessment Units - top of page
The purpose of admissions to Geriatric Assessment Units is to provide seniors with a complete health assessment. The geriatric assessment unit teams work with seniors and their families to establish appropriate goals and therapeutic plans. The team makes recommendations to help seniors remain as independent as possible. The Geriatric Assessment Units are located at the Ottawa Hospital, Civic Campus and at the Queensway-Carleton Hospital.
Geriatric Day Hospitals - top of page
The purpose of Geriatric Day Hospitals is to provide multidisciplinary assessment for patients who are experiencing a change in function, memory, mood or have complex medical issues. Short-term treatment, counseling, and education are available to patients and their caregivers to facilitate community support and long-term care planning. There are four Geriatric Day Hospitals, they are located at the Sisters of Charity Organization, Elizabeth Bruyère Pavilion and Bronson Pavilion, the Ottawa Hospital, Civic Campus and the Queensway-Carleton Hospital.

Geriatric Outpatient Clinics - top of page
Geriatric Outpatient Clinics are clinics staffed by physicians and nurses who are specialists in the assessment of elderly persons.

Geriatric Rehabilitation Units - top of page
Geriatric Rehabilitation Units are hospital beds where elderly people may be admitted for individualized rehabilitation programs under the direction of a multidisciplinary team.
