Eligibility

• 65 years of age/over (referrals for younger individuals presenting with geriatric problems that may benefit from specialized geriatric services are evaluated on an individual basis),
  
  
• Residents of Ottawa-Carleton or the surrounding areas,
  
  
• Patient agrees with referral
  
  
• Family MD agrees with referral
