Eligibility
- 65 years of age/over (referrals for younger individuals presenting with geriatric problems that may benefit from specialized geriatric services are evaluated on an individual basis),
- Residents of Ottawa-Carleton or the surrounding areas,
- Patient agrees with referral
- Family MD agrees with referral
