For seniors living at home, the first step in accessing the services of the RGP is by contacting one of our Outreach Teams.

The Outreach Teams operate out of two different sites. They can be reached by calling:

For seniors who are currently hospitalized, access to our services can be obtained through our geriatric consultation services available in all local hospitals.
