GERIATRIC OUTPATIENT CLINICS
INTRODUCTION
Geriatric Outpatient Clinics are specialized clinics
designed to provide comprehensive geriatric assessment to seniors
living in rural communities outside of Ottawa Carleton.
When to Refer:
- For assessment of common geriatric problems such as falls/impaired
mobility,
confusion, depression, polypharmacy, dizziness, Parkinsons
Disease and
unexplained functional decline.
- For assessment of the interaction of multiple and complex medical
problems.
- The interaction of medical and physical needs with the needs
of the caregiver (i.e. caregiver stress, placement being considered.)
Referral Mechanism
A Physician referral is required. However family members, CCAC workers
etc. can suggest and discuss with the physician if a referral can
be initiated.
