GERIATRIC OUTPATIENT CLINICS

INTRODUCTION

Geriatric Outpatient Clinics are specialized clinics designed to provide comprehensive geriatric assessment to seniors living in rural communities outside of Ottawa Carleton.

When to Refer:

  1. For assessment of common geriatric problems such as falls/impaired mobility,
    confusion, depression, polypharmacy, dizziness, Parkinson’s Disease and
    unexplained functional decline.
  2. For assessment of the interaction of multiple and complex medical problems.
  3. The interaction of medical and physical needs with the needs of the caregiver (i.e. caregiver stress, placement being considered.)


Referral Mechanism
A Physician referral is required. However family members, CCAC workers etc. can suggest and discuss with the physician if a referral can be initiated.
