Who Benefits
- Seniors, typically 75 years of age or more, with complex, multiple health problems, and who depend, or are at risk of depending upon others for activities of daily living. This includes 15% of the population 65 years of age or more, sometimes referred to as the frail elderly,
- Family caregivers, through education are more able to cope with specific problems, and are linked with appropriate community services.
- Society, through the reduction of unnecessary human and financial costs associated with many acute and chronic illnesses more commonly affecting seniors.
