Specialized Geriatric Services available through Central Intake

Geriatric Assessment Outreach Teams ​

A Geriatric Assessor completes a comprehensive geriatric assessment on adults who are 65 years or older.

Older adults are referred by their Primary Care Provider, other healthcare professionals, family, caregivers or themselves. Referrals are accepted for people living in the community. Referrals cannot be accepted from long-term care facilities or out of province.

Our Outreach Teams are committed to working with the Primary Care Provider and community services agencies.

Geriatric Clinics & Day Hospitals

A physician referral is required.

Geriatric Clinics and Day Hospitals are out-patient programs that provide multidisciplinary assessment and/or treatment for older adults experiencing a change in function, memory, mood, mobility or have complex medical issues. Treatment, counselling and education are available to older adults and their caregivers to optimize health and independence.

Before an appointment is made for a Geriatric Clinic or Day Hospital, many older adults have already been seen in their own home by the Geriatric Outreach Assessor.

Referrals may also be made through the Emergency Department Geriatric Emergency Management (GEM) Program by family physicians or from in-patient units.

GeriMedRisk

A virtual service designed to work directly with the older adult’s doctor, nurse practitioner or pharmacist to deliver clinical recommendations and educational materials within approximately 5 business days.

Ontario clinicians can contact the GeriMedRisk interdisciplinary specialist team of pharmacists and doctors in geriatric psychiatry, clinical pharmacology and geriatric medicine for advice regarding how medications may affect their older patient’s dementia, physical and/or mental health conditions. As part of the consult, GeriMedRisk contacts the older adult or their caregiver by phone to create an updated medication list.

In collaboration with Specialized Geriatric Services, GeriMedRisk is accessible to clinicians through the Central Intake for SGS referral form. Additionally, GeriMedRisk is available across the province by phone, fax, or eConsult.

Montfort Geriatric Services

A bilingual multidisciplinary Specialized Geriatrics Clinic is held once a week. Patients typically attend 1 – 3 sessions for a full Geriatric workup by a Geriatrician and other multidisciplinary staff as appropriate.

Referrals can be made by the Geriatric Outreach Assessment team, internally from Montfort Hospital, or from the community by Family Physicians.

Geriatric Psychiatry Community Services of Ottawa (GPCSO)

The mission of GPCSO is to provide expertise and leadership in the practice of community-based geriatric psychiatry, using an interdisciplinary approach, with the aim of enhancing the mental health and well-being of the elderly in their own environment.

Referrals are accepted by mail or fax from physicians, community agencies, health care professionals, families, friends or the individual concerned.

Geriatric Emergency Management (GEM)

The GEM Nurse sees people over 65 years of age who come to the Emergency Department who have been identified with special concerns. They are nurses with special training in geriatrics and they can make recommendations for support at home or follow up with specialists if required.

The Geriatric-Psychiatry Service

The program provides a range of services to meet the mental health needs of adults 65 years of age and over.

Working together with families, primary care physicians, community psychiatrists and community agencies, staff help individuals improve their quality of life and achieve optimal level of functioning.

The Geriatric-Psychiatry Service provides mental health consultations for people aged 65 and over, their families and caregivers.

Mental Health Service Renfrew County

Adults 65 years of age and older experiencing mood, personality or behavioural changes, who are in need of psychiatric evaluation and consultation.
